THERE IS A CHANGE COMING TO YOUR DOCTOR'S SURGERY: 'CARE NAVIGATION' - What does it mean to you as a patient?

Please see Further Information for full details

*Pre-bookable appointments now available as a face to face consultation or as a telephone consultation****

***We ask all our patients who are eligible for a flu vaccination to telephone us and book in as soon as possible, we have Saturday morning appointments available***

Glan Rhyd Surgery now has a waiting room blood pressure monitor. Why not call by, have your blood pressure recorded at your own convenience and hand the printed reading to a member of the reception team.

We are pleased to inform all our patients and patients registered elsewhere, that we now provide IUD/IUS (coil) insertions and removals. Please contact the surgery for further information.

HOW DO I REGISTER WITH THE PRACTICE ?

All patients must complete a new patient questionnaire available from reception and must also reside within our practice boundary. We would ask all patients who are prescribed repeat medications, ensure they have a 28 day supply of medication before registering. reception.jpg

When you register as a new patient you will be invited to attend for a short consultation with our Health Care Assistant. It is important that we know about relevant medical history while we await receipt of your medical records from your previous GP. This consultation completes your registration with this practice.

You can register with a GP of your choice within the practice but as we are a group practice this does not however guarantee that you always see the doctor of your choice.
