****UPDATE APRIL 2020****We are aware that some people are feeling worried about contacting their GP Surgery for advice and care, or contacting them much later than they would normally if they are unwell.

We want to assure you that your GP Surgery still expects and wants to hear from you if you need them- not just for Coronavirus related symptoms.

If you have any concerns please don’t hesitate to telephone your surgery, but please don’t turn up there in person. You will be offered a telephone or video call with the relevant Health Professional and only if they feel it is necessary, you will be invited in for further assessment.

If you have symptoms of Coronavirus and need to be seen, this will be in a designated area.

**Due to the Corona Virus outbreak we are currently dealing with patients via a telephone triage system in the first instance

**Please do not attend the surgery under any cirucmstance  if you have any of the following symptoms :-  high temperature, new cough, shortness of breath or breathing difficulties, in this instance please follow the guidance below.

We are pleased to inform all our patients and patients registered elsewhere, that we now provide IUD/IUS (coil) insertions and removals. Please contact the surgery for further information.

Care Navigation

The purpose of Care navigation is for patients to receive care from the most appropriate professional:

  • Care Navigation is a person-centred approach that uses information to help ensure your individual needs are met
  • It enables staff to effectively provide you with information about how to access alternative local health
  • Care Navigators can support you to access the most appropriate service, which may not need to be a GP
  • care Navigators understand that what you tell them may lead to you still needing to see a GP

GP Practice receptionists have been trained as CARE NAVIGATORS and have the full support of GPs.

You will be politely asked for the reason for your appointment request as CONFIDENTIALLY as possible,

CARE NAVIGATION means not all visits to your Surgery will result in you being offered a GP appointment.

You may be directed to support from VOLUNTARY ORGANISATIONS/COMMUNITY BASED SERVICES depending on your issue

You may be offered CARE from other members of the Surgery team, such as a PHARMACIST or PRACTICE NURSE

When you present with symptoms that meet the criteria for other services such as physiotherapy, pharmacy, optician or third sector service; the CARE NAVIGATOR can offer you a choice and give you information to help you get to the most appropriate help.

Please be patient with the Care Navigator (Receptionist) as the best way to help you get to the most appropriate support is by asking you specific questions.

The services we will be offering to you are:-

The Dentist

The optician

The Direct Access Physiotherapy Department at YAB

The Minor Injuries Department at YAB

Your local Pharmacy

Gwent Mental health Consortium
