This online form can be used to request a routine/prebookable appointment. Routine appointments are released each Wednesday, at 10am for the whole of the following week. You can submit this appointment request form anytime and you will be sent a text message or receive a phone call, informing you when your appointment will be.

This form MUST NOT be used for clinically urgent appointment requests, urgent prescription requests or life-threatening emergencies. 111 is available in such cases and when we are closed.
