New Patients
Register as New Patient
All patients who reside within our practice boundary can register by completing a new patient questionnaire which is available directly from the practice or can by requested to be sent by post, using our email address of correspondence.w93032@wales.nhs.uk. We would ask all patients who are prescribed repeat medication, have a 28 day supply of medication before registering and hand in a copy of their repeat order form to the practice.
When you register as a new patient you will be invited to attend for a short consultation with our healthcare assistant. It is important that we know your relevant medical history while we await receipt of your medical records from your previous GP. This consultation completes your registration with this practice.
Practice Area
Our practice area centres on Ebbw Vale. To the west we cover Tredegar and Waundeg. To the south, our boundary ends at Ysbyty Aneurin Bevan (meaning we can't accept new patients from Victoria or Waunlwyd). To the west, our boundary ends at the top of Beaufort Hill, and in line with the A467, meaning Brynmawr and Nantyglo are not within our boundary. And to the north, we cover Garnlydan and Rassau but stop before the moors. Our boundart map is displayed at the bottom of this page.
Any patients who move address to an area outside of our boundary will be asked to register in a different surgery. We understand that some patients have concerns about changing surgeries but will ensure this process is as simple as possible for all and endeavour to help patients locate a new surgery if requested.
If you live outside these boundaries you will not be allowed to register at Glan Rhyd Surgery. A list of alternative practices can be obtained here.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
